Confidentiality & Privacy Policy

The law protects the relationship between a client and a psychotherapist and information cannot be disclosed without written permission.

Exceptions

  • Suspected child abuse or dependent adult or elder abuse, for which we are required by law to report to the appropriate authorities immediately.
  • If a client is threatening serious bodily harm to another person/s, we must notify the police and inform the intended victim.
  • If a client intends to harm himself or herself, we will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, we will take further measures without their permission that are provided to us by law in order to ensure their safety.

Collecting and Using Personal Information
Welcome to the MOSAIC FAMILY Counseling Center Inc. website. We appreciate your interest in us! You may impart information you don't consider confidential and it will be placed in your file. If you send an email regarding an appointment, we may call or confirm that appointment via email.

Where you provide information to us using the "Contact Us" form, you are advised that this is an ordinary email we receive with the information you type in. For your reply, please indicate whether you wish us to respond to you by telephone or by your e-mail address (please note that such an e-mail reply is not delivered in a secure server environment, hence not private, see notes about regular e-mail, above). We follow the rules of HIPAA and will not send confidential information over the unsecured email system.
Beyond answering your question, website management personnel will periodically review questions (without patient identifiers or e-mail addresses) to determine if additional information should be placed on the website that would benefit other patients.
